Upstate Physical Therapy encourages you to know your insurance benefits and coverage. As a courtesy to our patients, we will file insurance claims on your behalf. Please understand, however, that the financial responsibility for treatment we provide remains with you. If your insurance plan is unable to make a timely payment for our services, we may request your assistance to follow up with your insurance company. Please make payments for all patient co-pay, co-insurance or deductible amounts at the time your treatment is provided.
